All children must complete:
Medical Examination (Infant/Toddler/Preschool)
All school aged children (kindergarten and above) must complete:
School-Age Child Health Report
All children less than 2 years old must complete:
Infant and Toddler Feeding and Care Plan
We are also required to obtain a copy of your child’s current immunization record. The state of Missouri requires specific immunizations. Click HERE to view the state of Missouri requirements.
